The risk assessment shows the likelihood and severity when plans to mitigate the risk are not in place giving the risk score. There should be another box with the residual risk score after the mitigation steps are followed to bring the risk score down if the risk assssment is followed. Please add this box otherwise it looks like everything is a HIGH RISK when CQC checks and this is not what the risk assessment is supposed to do.
